This is a professional public health nursing position with an emphasis on the health of both individuals and aggregate groups within the community. The nurse in this position provides nursing care, health education, and advocacy to adults and children in the program areas of Maternal Child Health and Communicable Disease Control.
Works under the supervision of the Clinical Services Manager.
Does not supervise other staff members
ESSENTIAL DUTIES AND RESPONSIBILITIES
(A) Provides nursing care to patients in the following areas: Maternal and Child Health, Communicable Disease Control and Community Health. Administers immunizations and medication to children and adults; performs and evaluates laboratory testing; arranges patient referrals; performs nursing assessments in patients' homes, community sites, and also in the clinical setting; i.e., HIV, STD, Lead, Childhood and Community Immunization, and Tuberculosis clinics.
(B) Participates in patient and community education; provides health education in the clinics, the community and the home; presents health education programs to students and health care professionals, civic groups, and human service agencies; evaluates the effectiveness of the education.
(C) Provides health education and resource and referral information to child care facilities.
(D) Conducts child care inspections in conjunction with Environmental Field Services staff.
(E) Collaborates with Environmental Field Services staff in assessing home environments of children and adults; makes appropriate referrals and acts as a facilitator to assure services.
(F) Receives referrals from social agencies, schools and health care providers for in-home assessments of children and adults at risk.
(G) Provides assessment, identifies problems, establishes a plan of action, makes referrals, and provides support and education in a case management format.
(H) Develops and implements community health promotion activities.
(I) Provides screening and community immunizations.
(J) Assists in the development of program goals and objectives and submits monthly and quarterly statistical reports as needed.
(K) Serves on committees, councils and task forces based on area of expertise.
(L) Plans, implements, and evaluates program objectives and activities related to designated area of expertise.
(M) Assures patient data are collected and entered into computer databases as required.
(N) Provides quality and effective customer service with courtesy and under-standing to our customers, citizens and internal departments.
(O) Works as a team player with peers and other professionals within and outside of the agency.
(P) Available to respond to public health emergencies and assume the role designated for him/her in the public health emergency response plan.
(Q) Participates in continuing education to enhance competence in public health.
(R) Acquires, maintains and demonstrates the public health core competency Skill levels listed below:
i. Aware - Policy Development / Program Planning, Financial Planning, and Management
ii. Knowledgeable - Analytical Assessment, Community Dimensions of Practice, Basic Public Health Sciences, Leadership and Systems Thinking
iii. Proficient - Communications, Cultural Competency
(S) Performs related work as may be required.
Education and Experience:
(A) Bachelor of Science degree in Nursing from a college or university accredited through the National League of Nursing program with one year of community health nursing experience desirable.
(B) Incumbent is required to possess a license as a Registered Nurse in the Commonwealth of Pennsylvania.
Necessary Knowledge, Skills and Abilities:
(A) Ability to speak Spanish or other languages desirable.
(B) Ability to apply principles of health promotion and disease prevention.
(C) Ability to provide competent, high quality nursing care in a variety of settings.
(D) Ability to make independent nursing decisions based on nursing assessment.
(E) Working knowledge of methods and techniques used in the prevention of Communicable Disease, Chronic Disease and in Maternal and Child Health.
(F) Ability to establish and maintain effective working relationships with community partners, health care professionals and the general public.
(G) Willingness to work as a team player with peers and other professionals within and outside the agency.
(H) Ability to work effectively with individuals of diverse ethnic, racial and socioeconomic backgrounds.
(I) Skill in public speaking and ability to write clear, concise progress notes in clients' health/medical records.
(J) In-depth knowledge of the functions of other health and social agencies and their relationship to public health programs.
(K) Knowledge of basic computer skills needed to create documents and presentations and to analyze data.
(L) Possession of a valid driver's license.
(M) Act 153 of 2014 background checks required.
CPR/First Aid Certified
