This is a professional public health nursing position with an emphasis on the health of both individuals and aggregate groups within the community. The nurse in this position provides nursing care, health education, and advocacy to adults and children in the program areas of Communicable Disease Control and HIV Navigation.
Works under the supervision of the Communicable Disease Manager.
Does not supervise other staff members.
ESSENTIAL DUTIES AND RESPONSIBILITIES
(A) Provides HIV navigation Services and care to patients in the following areas: Communicable Disease Control, HIV Navigation; performs and evaluates laboratory testing; arranges patient referrals; performs nursing assessments in patients' homes, community sites, and also in the clinical setting, e.g., STD and HIV clinics.
(B) HIV Navigation Services (HNS) responsibilities: include guiding clients to health care systems, assisting with health insurance and transportation, identifying, and reducing barriers to care, and tailoring health education to the client to support their full engagement in care.
(C) HIV Navigation skills includes client identification, health education, motivation, guidance to services, appointment assistance, cultural sensitivity, guidance while in services for retention, adherence support, and assistance with transportation.
(D) HNS programming is implemented in accordance with the HIV Grant deliverables required by the PA Department of Health.
(E) Activities include seeking out and engaging high risk individuals in order to enroll and assist them with HIV navigation services.
(F) Collects and analyzes population-based health information.
(G) Identifies Populations at-risk and adjusts goals and implementation of HIV Navigation services to optimize program impact.
(H) Implements educational programs with individuals, groups, and human service agencies; evaluates the effectiveness of the education.
(I) Collaborate with community leaders to promote preventive and routine health care services in their community.
(J) Documents HNS activities and PA Dept. of Health reports in an accurate, detailed, and timely manner.
(K) Participates in activities designed to improve community as well as public health emergency preparedness.
(L) Participates in patient and community education; provides health education in the clinics, the community, and the home; presents health education programs to students and health care professionals, civic groups, and human service agencies; evaluates the effectiveness of the education.
(M) Provides assessment, identifies problems, establishes a plan of action, makes referrals, and provides support and education in a case management format.
(N) Develops and implements community health promotion activities.
(O) Assists in the development of program goals and objectives and submits monthly and quarterly statistical reports as needed.
(P) Serves on committees, councils and task forces based on area of expertise.
(Q) Plans, implements, and evaluates program objectives and activities related to designated area of expertise.
(R) Assures patient data are collected and entered into computer databases as required.
(S) Provides quality and effective customer service with courtesy and understanding to our customers, citizens, and internal departments.
(T) Works as a team player with peers and other professionals within and outside of the agency.
(U) Available to respond to public health emergencies and assume the role designated for him/her in the public health emergency response plan.
(V) Participates in continuing education to enhance competence in public health.
(W) Acquires, maintains, and demonstrates the public health core competency Skill levels listed below:
i. Aware - Policy Development / Program Planning, Financial Planning, and Management
ii. Knowledgeable - Analytical Assessment, Community Dimensions of Practice, Basic Public Health Sciences, Leadership and Systems Thinking
iii. Proficient - Communications, Cultural Competency
(X) Performs related work as may be required.
Education and Experience:
(A) Bachelor of Science degree in Nursing from a college or university accredited through the National League of Nursing program with one year of community health nursing experience desirable.
(B) Experience with public health, case management and HIV programming desirable.
(C) Incumbent is required to possess a license as a Registered Nurse in the Commonwealth of Pennsylvania.
Necessary Knowledge, Skills and Abilities:
(A) Ability to speak Spanish or other languages desirable.
(B) Ability to apply principles of health promotion and disease prevention.
(C) Ability to provide competent, high quality nursing care in a variety of settings.
(D) Ability to make independent nursing decisions based on nursing assessment.
(E) Working knowledge of methods and techniques used in the prevention of Communicable Disease, HIV Navigation, and Clinical Services.
(F) Ability to establish and maintain effective working relationships with community partners, health care professionals and the general public.
(G) Willingness to work as a team player with peers and other professionals within and outside the agency.
(H) Ability to work effectively with individuals of diverse ethnic, racial and socioeconomic backgrounds.
(I) Skill in public speaking and ability to write clear, concise progress notes in clients' health/medical records.
(J) In-depth knowledge of the functions of other health and social agencies and their relationship to public health programs.
(K) Knowledge of basic computer skills needed to create documents and presentations and to analyze data.
(L) Possession of a valid driver's license.
CPR/First Aid Certified
